Many parents are alarmed to find cavities in their baby’s first teeth, often exclaiming, “My baby was born with rotten teeth!” But dental experts warn that this assumption is misleading. Pediatric Dentist Dr. Nurgül Demir explains that babies are not born with decayed teeth. Instead, early exposure to decay-promoting factors or developmental issues in enamel can create the illusion of pre-existing cavities.

“Some developmental defects or early bacterial exposure can damage enamel before a child even learns to chew,” says Dr. Demir. “These may show up as yellow-brown discolorations that resemble cavities, but they’re actually early warning signs — and often preventable.”

Structural Deficiencies May Be Mistaken for Cavities

Tooth enamel mineralization defects or enamel hypoplasia — where the enamel is weak or thin — are common causes of abnormal-looking baby teeth. These can result in visibly discolored or fragile teeth that many parents assume are decayed. However, true cavities are primarily caused by poor oral hygiene, improper feeding habits, and bacterial activity. That’s why early dental consultation is key to differentiating between structural issues and real decay.

Bottle Decay Can Erode Baby Teeth Rapidly

Parents often describe their baby's tooth damage with phrases like “The teeth just crumbled” or “They disappeared overnight.” Dr. Demir explains that this is usually a result of bottle decay — a form of early childhood caries caused by prolonged exposure to sugary liquids in bottles. Nighttime bottle feeding is especially risky, as it leaves milk sugars on teeth for hours, fueling bacterial acid attacks that erode enamel.

Oral Hygiene Should Begin with the First Tooth

The first baby tooth usually erupts between 6 and 12 months of age. Dr. Demir stresses that oral hygiene should start from the moment that first tooth appears. “Parents should begin brushing with a tiny amount of fluoride toothpaste — no more than a grain of rice — and continue daily brushing as more teeth emerge,” she advises. These habits, combined with routine dental checkups, can significantly reduce cavity risk.

How to Protect Your Baby’s Teeth from Decay

To ensure long-term oral health, Dr. Demir outlines essential tips for parents:

  • Use separate feeding tools for your baby. Never share spoons, pacifiers, or bottles with others to avoid transmitting decay-causing bacteria.

  • Limit adult-baby mouth contact. Avoid kissing your baby on the lips or using the same utensils, which can transfer bacteria directly to your child’s mouth.

  • Avoid sweetened substances. Do not dip pacifiers in honey or molasses, and avoid sugary drinks in bottles or sippy cups.

  • Clean your baby's mouth after breastfeeding. Wipe the gums and teeth with a soft, damp cloth or give a small amount of water to rinse milk residue away.

  • Don’t skip dental visits. A baby’s first dental exam should take place within six months of the first tooth eruption or by age one. This allows the dentist to assess risk factors and begin preventative care early.

Early Action Prevents Bigger Problems Down the Road

By addressing oral health early, parents can avoid future dental treatments that are both costly and emotionally taxing. “Cavities are the result — not the starting point,” Dr. Demir reminds us. “If we understand and eliminate the causes, we protect not just baby teeth but long-term oral health.”
